Obesity: A Global Epidemic

Obesity is a complex metabolic disorder, caused by an imbalance in energy homeostasis.  Obesity is a worldwide problem and poses one of the greatest challenges to medicine, especially as the prevalence of obesity has increased steadily over the last few decades.

The World Health Organisation estimated that in 2015, 2.3billion adults will be overweight and another 700 million or more will be obese.  In Australia, 62-65% of adults are currently estimated to be overweight or obese.  Young women in the 25-35 year age group are putting on weight faster than other age and sex cohorts, and there are concerns about the increased incidence of childhood obesity.

Weight loss programs based on lifestyle modifications (low-calorie diets and exercise) are of limited benefit, achieving a mean weight loss on average of less than 5% over a 12 month period.  Surgical methods for weight loss, including laparoscopic band placement and gastric bypass procedures are invasive and may be associated with significant morbidity and mortality.

An alternative approach, managed endoscopically, is the Orbera weight loss system.  This involves the placement into the stomach of a soft, silicone intragastric balloon, expanded into a sphere by infusing up to 700ml of saline through a self-sealing valve.

The Orbera system induces early and prolonged satiety with only a small meal, and is designed to remain in situ for a maximum of six months, before the saline is removed and the deflated balloon withdrawn endoscopically.  When used in conjunction with a long-term lifestyle program, including a balanced, low-caloric diet and exercise, significant weight loss is routinely achieved – well in excess of that often documented with diet and exercise alone.
